This form helps the filmmaker write notes if there's anything needed as it goes such as batteries for the camera extra lighting if needed and perhaps even extra mikes 

Some notes about the environment in my case it's about COVID-19 and that everyone will follow the government guidelines if filming ends up happening. Everyone should keep a safe distance of 6 feet if prompted masks should be on unless in the scene.
